The IP Justification form is a requirement from ARIN, which is regularly audited. Each form submitted will be manually reviewed for accuracy. You must provide valid data to receive additional IP allocations.

The IPv4 request form is split up into four sections.

Requested Subnet Size: From this drop-down, select the IP block size that you’re requesting, which is in addition to any IP blocks you currently have assigned to you.

/29 - 8 IPs, 5 Usable
/28 - 16 IPs, 13 Usable
/27 - 32 IPs, 29 Usable
/26 - 64 IPs, 61 Usable
/25 - 128 IPs, 125 Usable
/24 - 256 IPs, 253 Usable

Existing Assignments: You must provide details about IP blocks that you have been previously assigned. For each IP, you must list the customer that the IP is assigned/re-assigned to, what the IP is being used for, and a domain/hostname if applicable. The customer must be either a valid full name or a company. The usage should be something that requires its IP and a service/usage that cannot be shared with any other IP due to technological limitations. For any IPs that are not usable, please either list not usable or N/A.

What do you need to tell us? Please read all the information below.

1) You have 5 IPs with your server, you have to tell us in the form in the same format which is written in the Existing Assignments box.

Above is the example you have to edit with your information.

1) IP: you have to replace all existing IPs which is with your server IPs that you got.
2) Customer: Should be your name or the name who is responsible for these Additional IPs.
3) Usage: Let us know the usage of your existing IPs which is with your server right now. For what purpose will you use IPs?
4) Domain/Hostname: You should have a qualified domain name or any hostname that is related to you or your company.

Move on 2nd faze.

Remember that your subnet is xxx IPs and you have to divide all xxx IPs in IP-based Virtual Hosting option and Other (specify) option.

1) IP-based Virtual Hosting: You need to tell us how many IPs will be used for Virtual Hosting.
2) Other (specify): You need to tell us how many IPs will be used for Other (specify)
3) SSL Virtual Hosting: If you will use IPs for SSL Virtual Hosting, you need to tell us how many IPs.

Move On 3rd faze.

1) Reason for Using IP-based Virtual Hosting: You have to tell us a valid reason in detail for which purpose or what your goal is for the usage of IPs.
2) Other (specify) Address Utilization: You have to tell us a valid reason in detail for which purpose or what your goal is for the other IP's usage.
